Child Disability Team

The Child Disability Team is a specialist team who look after the specific needs of children and young people with disabilities. It is one of the services provided in Neath Port Talbot that works with disabled children and their families with a view to providing services to assist in minimising the effects of a child’s disability and to enhance their well-being, and families to meet the needs of their child, with a disability.

These needs can be due to:

  • Learning or physical disability
  • Chronic illness
  • Sensory disability

The team consist of Social Workers, Practice Support Workers, an Occupational Therapist and Behaviour Support Worker. The Team work with a number of Professionals to promote and safeguard the well-being and welfare of disabled children and young people.

Following an assessment of need, the Child Disability Team can provide services that include:

  • Carers Assessment
  • Specialist play through our Portage service for pre-school children
  • Direct Payments
  • Respite Services
  • Behavioural Support
  • Access to Specialist Needs Groups
  • Referral to Young Carers for siblings
  • Information on Early Intervention Panel, and Specialist support groups in the community if Statutory services are not appropriate
  • Support if necessary until the child attains the age of 18 years
  • Transition to Adult Services if required

The purpose of the assessment is to gather information and to identify the needs of the child or children and/or their family and the nature and level of support.
