It brings together a range of people and agencies who work with the family as a whole.
We provide a service to families where:
- There is a family member 0-25 yrs
- Difficulties have been identified that require help from 2 or more agencies e.g. school, health, housing.
- There is no social services involvement
You can be referred to TAF by another professional that knows your family such as a Teacher, GP, Health Visitor, Youth Worker or you can refer yourself.
The aim is to support families to make the changes needed to improve their lives and live independently within their own communties.

Referrals for all services can be made through the Single Point of Contact (SPOC) 01639 686803
spoc@npt.gov.uk and are available to families who do not need support from Social Services. All referrals are passed to a weekly referral panel which will help make sure that families get support from the service that best meets their needs.
