Community Resource Team
The Community Resource Team (CRT) is a joint service provided by the Abertawe Bro Morgannwg University Health Board (ABMU) and Neath Port Talbot Council Social Services. The CRT provides help to adults (over the age of 18) living in Neath Port Talbot who need support to stay independent, within their own homes.
The CRT offers an improved service that ensures patients receive the right intervention, at the right time, from the right professional. It simplifies the process by coordinating both health and social care needs.
It is easy to access the CRT. There is one common access point for people needing support from the service. This is called the Neath Port Talbot’s Adult’s and Children’s Single Point of Contact Team (SPOC)
The Sensory Support Team is a service to people who are Visually Impaired, Deaf or Hard of Hearing, or are Deafblind (or dual sensory loss - a significant combination of hearing and sight loss). We support both children and adults with a sensory disability.
Our services might be the only support you need or they might form part of a care package which includes other services from the CRT.
We might also work alongside other services or groups to try to achieve the best possible outcome for you.

Community Reablement is a short-term health & social care support service. It aims to help you live independently at home for as long as possible.
The service aims to encourage and support you to learn or re-learn the skills needed for daily living. This might be after an illness, an accident or a stay in hospital.
Community Reablement can:
• Help you to recover from an illness or a stay in hospital
• Rebuild your confidence
• Promote independence
• Help you decide about your long term care needs
Community Reablement will work with you to identify what you can do for yourself and what is causing you difficulty. It will discuss with you what you want to achieve.

The Community Occupational Therapy Service aims to maintain, promote and restore independence for people of all ages, so that they are able to live more productive and enjoyable lives.
The service aims to promote your health and wellbeing through everyday activities.
By using various techniques, adapting your environment and using specialist equipment, we can work with you to:
- improve your quality of life
- increase your independence and safety
- enable you to access other support services
- increase your choice and control

The Assistive Technology Service offers a range of categories to enable people to remain safe, secure and independent in and around their own home.
Assistive Technology:
- offers simple to use equipment
- offers peace of mind for the client, carers and family/friends
- can reduce the level of support required to remain living at home
- 24 hour/365 day response from the monitoring centre
- promotes independence
- reduces risks
