Information you may need to provide

During your appointment

If available, we need to know certain information on the person who has died. This includes their:

  • full name, including any other known names
  • marriage or civil partnership certificates
  • date and place of birth
  • occupation
  • address
  • spouse or civil partner's full name and occupation (if applicable)

We also need to know:

  • when and where they died
  • the name of the funeral director conducting the funeral
  • whether they are being buried or cremated

Corrections to death registrations

Check all information is correct before signing the register. Errors found after registration will need official correction and incur a statutory fee.
